    2. Tangible Personal Property Memorandum

    3. Declaration of Disposition of Last Remains and Funeral Arrangements

    4. Durable Power of Attorney for Property

    5. Medical Durable Power of Attorney (Who do you want making medical decisions on your behalf if you are ever physically or mentally unable to communicate your desires to the attending physician?)

    6. Advance Directive for Medical/Surgical Treatment (Living Will) (What is your preference regarding life-sustaining medical technologies?)

    7. HIPAA Authorization Form

  • Our Signature Revocable Trust Plan is for an individual with a Net Worth of up to $1.2 million (or combined marital Net Worth up to $2.4 million).

    1. Trust Agreement

    2. Certification of Trust

    3. Statement of Authority

    4. Trust Funding and Beneficiary Designation Instructions

    5. Deed Preparation and Recordation with Clerk & Recorder for all Colorado properties (out-of-state properties will need to be transferred into the Trust with the assistance of an attorney licensed to practice in that state)

    6. Pourover Last Will & Testament (this Will would only be necessary if some property subject to probate was not properly transferred into the Trust “res” prior to death)

    7. -12. Preparation of Items 2 through 7 listed in the Signature Will Plan, above.

  • Our Complex Marital Trust and Will Plan is for combined marital Net Worth up to $5 million

    • Complex Wills or Revocable Trusts for married couples with Tax Planning: Portability, Bypass, or QTIP Trusts
